Community Teen Coalition, Inc. is proud to play a role in helping individuals and families who have experienced a loss of income or food related to the COVID- 19 pandemic. Click here for FAQ and to review flyer for a list of criteria and eligibility to begin the application process.
Important Notes: The forms below are being provided for information use only at this point. Community Teen Coalition office are temporarily closed to walk-in clients so someone from the office will contact you to begin the telephone screening process. Do not drop off any documentation unless you have been requested by a Community Teen Coalition worker to do so.
To be eligible for support, individuals/households must reside in Clayton, S. Fulton, S. DeKalb, or Spaulding County residents with an income at or below 100 percent of Area Median Income (AMI) and have a need for any of the following assistance:
Area Median Income is determined by the U.S. Department of Housing and Urban Development. The estimate AMI for the communities served is $59,850 for a four person family (100 percent AMI).
Individuals/households must also demonstrate a loss of earned income related to the COVID-19 pandemic due to medical reasons (incubation directive), business closure or layoff or school closures.
Each household may be eligible for up to $2,000 in emergency funding.

STEP 1: Complete the GENERAL APPLICATION
STEP 2: Complete the CTC FINANCIAL ASSISTANCE FORM
STEP 3: Complete other applications pertaining to need:
STEP 4: Submit all necessary documents (lease agreement, bill, etc.)
STEP 5: Once all documents have been received, you will then be assigned a CTC case manager, who will review the completed forms via email (alternative arrangements will be made for people who do not have access to email). You must complete and submit all pertinent forms for consideration. Please allow 24-48 business hours for a response, via email or phone.
